Hey Console Crunch users,
This is somewhat a rare problem. I will give you a list of reasons this may happen and then give a couple of solutions.
A few reasons may be -
An EBOOT. I had this problem with an SPRX menu and their EBOOT provided.
Random trophy corruption... Yes, it can happen.
Those are the only 2 I know of so please let me know of any other reasons this may happen.
Some Solutions -
Using FileZilla, multiman or another way to access your files VIA your IP, to delete your trophy file (DIR - dev_hdd0/home/yourprofile/trophy) delete the 'trophy' folder.
Replacing your EBOOT with a default one or an anti-ban one.
Removing your SPRX menu then trying to boot up BO2
